UTASA and FAM have entered into a trial joint venture for a period of three months. FAM has contracted with the Insurance companies listed below and would like for you to give them the opportunity to secure their clients accidents and with your assistance continue to deliver service excellence.
Members should note that after the three months trial period, UTASA will review the position through its membership and will in this period, also be negotiating better terms and rates for the membership with FAM. Implementation date is 3rd September 2014, members who have not signed and returned document will not enjoy the benefits until such time that they forward completed document with required attachments.

UTASA members interested in securing a position as a Service Provider are requested to verify their details by completing the following page and also attaching the following documentation
1. Copy of owners ID
2. Confirmation of banking details (cancelled cheque or bank letter)
3. If Vat registered copy of VAT certificate
